We need 1-2 more documents to make you an official Mayesh customer. Please fill out the appropriate forms below and send them back to us using the form at the bottom of this page.
We will review your submission and will reply within 3 business days (business hours: Monday-Friday 6am-3pm PST).
- If you have a LOCAL Mayesh, then we just need the appropriate resale/reseller certificate information as indicated for your state below.
- If you do not have a local Mayesh and you will be a SHIPPING customer, then we will need the resale information AND a signed copy of the Shipping Policy form.

- Georgia Sales Tax Certification of Exemption

- Washington Reseller Permit - Washington businesses must send a copy of your current Reseller Permit. We can accept paper or electronic copies.
- All Other States: If you do not see your state listed please upload/send a copy of your state resale license or equivalent (e.g. Sales Tax ID).

2. OPEN FILES, FILL OUT & SAVE TO YOUR COMPUTER
The quickest way to fill out the forms is to do so directly on your computer. Open the files that you save to your computer in Adobe Acrobat, click the "Fill & Sign", fill in and sign the documents, and then save the file with your changes. Here are the instructions if you need more help.
