Please fill out the appropriate forms below and send them back to us - FILL OUT THE FORM AWITH YOUR CUSTOMER'S INFORMATION.

You can skip to Step #3 to send in the forms that your customer sent you. Please ensure that each form is its own file.

 

1. DOWNLOAD YOUR FORMS

Use the links below to download your appropriate forms. For now, we only need your resale certificate and if you are are shipping customer, the signed shipping policy form.

CREDIT CARD AUTHORIZATION FORM

Please note, that you will not download the credit card authorization form here, but rather your Mayesh rep will email you a form to sign after inputting your credit card information into our system over the phone or in person. Once they enter the card information, the number is encrypted, and they will receive a token code. The form that your Mayesh rep will send you to sign contains the token and only the last 4 digits of your card number. 

2. OPEN FILES, FILL OUT & SAVE TO YOUR COMPUTER

The quickest way to fill out the forms is to do so directly on your computer. Open the files that you save to your computer in Adobe Acrobat, click the "Fill & Sign", fill in and sign the documents, and then save the file with your changes. Here are instructions if you need more help.
